Updates on Non-Emergency Medical Transportation and Community Health Improvement Plan lead agenda for Community Advisory Council’s June 5 public meeting

(Portland, OR) – The Community Advisory Council for Health Share of Oregon will hear updates on several of the Coordinated Care Organization’s (CCO) major programs during their monthly public meeting on Friday, June 5. The meeting, which begins at 12:30pm, will be held at Health Share’s offices at 2121 SW Broadway, Suite 200, in downtown Portland.

Council member Amy Anderson will provide an update from the Transit Advisory Committee (TAC) on Non-Emergency Medical Transportation. Anderson represents the Council on the TAC, a group of consumer advocates, transportation providers and CCO representatives whose role is to identify areas of improvement for the Ride To Care transportation program. TAC members, who represent the communities that Ride To Care program serves, regularly review transportation quality measures and provide feedback to Ride To Care administration.

Council member Sonja Ervin and Health Share Project Manager Sandra Clark will share a progress report on Health Share’s Community Health Improvement Plan (CHP). CCOs are required to conduct a Community Health Needs Assessment and implement a CHP based on that Assessment every three years. Health Share’s 2015 CHP focuses on behavioral health (mental health and substance use disorders) and chronic disease related to physical activity and nutrition, with resources focused on culturally-specific Community Health Workers and Peer Mentors.

The Council will also discuss Council membership updates, recruitment, and the officer elections process.

The meeting is open to the public and Health Share members are encouraged to attend.
