Behavioral health initiatives and programs reflect the collaborative efforts of Health Share's partners, as they work towards a single regional system, one in which mental health and addiction services are administered with standardized care management throughout the Tri-County Metro Area.

 

Connecting mental health patient to community resources (ITT) - Providing short-term intensive-care management and mental health services to engage high-risk patients in appropriate community-based support.  

Forensic Assertive Community Treatment (FACT) Program - Working with individuals as they leave the justice system to ensure they receive the mental health treatment and social service support they need.  

Children's Wraparound Program - Intensive, individualized care and support for children, focusing not simply on a child’s behavioral or mental health concerns, but also on the needs of the child’s entire support system, including caregivers, siblings, peers and communities.
