The ED (Emergency Department) Guide intervention is one of five interventions supported by the $17.3 million CMMI Health Commons Grant. The goal of this program is to connect individuals seeking care in the emergency department for low-acuity concerns back to primary care medical homes and support services for ongoing care. Using non-traditional workforce members to provide patient education and services navigation, this intervention aims to reduce individuals’ non-emergent ED use.

The ED Guides intervention recently received the results of a comprehensive program evaluation conducted by Providence’s Center for Outcomes Research and Education (CORE). The results showed that the team was especially effective when working with individuals who were newly enrolled in Medicaid and those who had four or more ED visits in the last year. In response to these results, the team shifted patient inclusion criteria to have the greatest potential for success among this vulnerable and costly patient group. To ensure smooth implementation of new intervention criteria, program leadership created and executed a communication plan to staff as well as internal and external partners.
