Welcome to Health Share of Oregon, a collective impact organization formed by 11 familiar and trusted organizations, including health and hospital systems and county mental health services: Adventist Health, CareOregon, Central City Concern, Kaiser Permanente, Legacy Health, Oregon Health & Science University, Providence Health & Services, Tuality Healthcare and Clackamas, Multnomah, and Washington Counties.

Together, we are creating a single integrated health care delivery system to transform care for Oregon Health Plan members in the Tri-County Metro area. Our goal is to achieve better health outcomes and better patient experience at lower cost.

We are committed to changing the things we know do not work:

  • To change the payment and incentive model that drives short “encounters” and productivity/volume to one built around producing value and better outcomes
  • To reduce administrative overhead and burden to providers
  • To dismantle the silos between physical health, dental health and behavioral health and replace them with new integrated efforts, where members are known, supported and guided by a team accountable for their care.
