일부 헬스 셰어 회원들은 주거, 식량 또는 기상 현상과 같은 큰 삶의 변화에 대처하는 데 도움이 되는 추가 혜택을 받을 수 있습니다. 이러한 혜택은 건강 관련 사회적 필요(HRSN)라고 합니다.

Nutrition Support

HRSN 혜택에는 다음이 포함됩니다:

  • 극한 기상 시 건강을 위한 가정 내 변화: 폭풍우, 폭염 또는 혹한 속에서 안전을 지키는 데 도움이 되는 장치들.

  • 주거 지원: 현재 거주 중인 주택에 계속 머무를 수 있도록 지원합니다.

  • Nutrition support: Help with food benefits and eating healthy.

도움을 받을 수 있을까요?

다음 조건을 모두 충족하는 경우 혜택을 받을 수 있습니다:

1. 귀하는 현재 오리건 건강 공유(Health Share of Oregon) 또는 오리건 건강 플랜(OHP)의 회원입니다.

2. 당신은 다음과 같은 사회적 욕구가 있습니다:

  • 식량이 부족하다

  • 월세 내는 데 어려움

  • 기후나 날씨로 인해 발생하거나 악화된 건강 문제

3. 다음 중 적어도 하나를 경험해 본 적이 있습니다:

  • You have current or past involvement in the child welfare system or foster care

  • 당신은 노숙자이거나 노숙자가 될 수 있습니다

  • 최근에 구금 상태나 정신건강센터에서 석방되었습니다

  • 귀하는 메디케이드 단독에서 메디케이드와 메디케어로 전환 중입니다.

  • 당신은 19세에서 26세 사이이며 특별한 건강 관리가 필요합니다.

4. 또한 특정 건강상의 요구 사항이 있어야 합니다. 예를 들어:

  • 복잡한 신체 건강 또는 치과 건강 요구 사항

  • 심각한 정신 건강 요구

  • 임신 중이거나 출산 후 12개월 이내

  • 만 6세 미만 또는 만 65세 이상

  • 응급실 방문과 같은 위기 서비스의 높은 이용률

  • 세수, 식사, 옷 입기 같은 일상적인 일에 도움이 필요합니다

  • 가정폭력 생존자

  • 발달 장애

About HRSN Housing Benefits

HRSN Housing benefits are not for emergencies.

If you are about to be evicted, contact your local Community Action Agency.

Not all members will qualify for HRSN housing benefits.

There are specific federal rules. You need to meet certain health, housing, and financial conditions to be eligible.

Requesting Housing Support

HRSN Housing supports can include help paying rent for up to 6 months, help paying for utilities like gas, electric and water, and other services for renters.

CareOregon manages HRSN Housing benefits for all Health Share members, regardless of their plan assignment.

You can request these benefits by one of these methods:

Staying healthy requires more than good health care. Food, shelter supports, and communication services that help you keep in touch with your care team are all key to your health.

Flexible services are items or services like temporary lodging, supplemental food, or a cell phone and service plan so you can keep receiving telehealth care from your provider. Our Care Coordinators can help you get temporary lodging, replace or supplement food, or get a cell phone and service plan so you can keep receiving telehealth care from your provider.

Flexible Services may be able to pay for items or services that are not part of covered benefits. Flexible Services are also known as Health-Related Services or Flex Funds. They are not the same as Health-Related Social Needs or HRSN benefits.

A copy of the Flexible Services Policy is available to download here.

Examples of Flexible Services might be:

  • Access to a warm pool to swim and exercise to help with your arthritis and improve your movement

  • A place to stay for a few days to recover from a hospital stay or surgery

  • A cell phone to keep you connected to your healthcare provider

To be approved, items or services must:

  • Not be a covered benefit under your health plan

  • Be cost-effective

  • Show a proven health benefit

  • Be part of your treatment plan or approved by your healthcare provider

  • Not be available through any other funding source
