Transitions of Care

How long does the change take?

You can keep getting care during the change:

  • For medical and dental care, you have 30 days.

  • For mental health care, you have 60 days.

  • Or, until your new doctor looks at your treatment plan.

Need to Switch from Health Share to Another CCO?

If you switch from Health Share to another Coordinated Care Organization (CCO), we’ll help make sure you still get the care you need.

If you have both Medicaid and Medicare, you get 90 days to keep getting care while you switch.

What Health Share Will Do for You

We’ll make sure you keep getting care and rides to your appointments, and can still see your doctor, even if they aren’t in the Health Share network. We’ll do this until:

  • You finish your treatment, or

  • Your doctor says you don’t need the treatment anymore.

If you’re seeing a specialist, another qualified doctor will check your treatment plan. Some types of care will continue with your current doctor until they’re finished:

  • Care before and after having a baby (prenatal and postpartum).

  • Transplant care for up to one year after the transplant.

  • Cancer treatment like radiation or chemotherapy.

  • Medicines that need to be taken for longer than the time listed above.
