Ride to Care gets many Health Share members to their medical and dental appointments.

What you need to schedule a ride
Please have this information ready before you call:
1. Your first and last name.
2. Your date of birth.
3. Your Health Share of Oregon Medicaid ID number.
4. Date and time of the appointment.
5. Address for pick up and drop off.
6. Clinic name, doctor’s name and doctor’s phone number.
7. Medical reason for the appointment.
8. If you need a one-way or round trip.

When to call
For emergencies, call 9-1-1 immediately
Call Ride to Care at least 48 hours before your appointment. You can call for a next day appointment, but urgent care appointments get priority. You can schedule up to 90 days before the appointment.
Call between 8am-5pm. Only urgent care trips can be scheduled after hours.
Call to schedule a ride to your medical appointment
Depending on your needs, you may get help paying for gas, tickets for the bus or MAX, or rides in a taxi or wheelchair accessible van.
Types of transportation
There are 3 types of transportation. Bus passes, taxi rides, and gas money for the mileage.
You can get a pass to take public transportation.
Most people live in the Tri-met service area, and get a Hop pass. The first time you schedule, you get a Hop pass in the mail. For future appointments, your trips will be loaded onto the Hop pass.
Plan ahead, it can take 5-7 business days to mail a Hop card or fare for other transit systems. If you have a Hop card, call 2 business days before your appointment to approve your trip and load your fare.

Focus card: a prepaid debit card
Gas money reimbursement
Ride to Care pays a per-mile rate for mileage to and from health care visits. You can drive yourself, or someone else can drive you. The money is loaded onto a Visa prepaid debit card for US Bank.
This is called a Focus Card.
Steps to get gas money reimbursed:
Get a Focus Card. The first time you call, they will create an account for a Focus Card with U.S. Bank. The card will be mailed from U.S. Bank, and could take 7-10 business days to arrive.
Call Ride to Care to schedule your trip before your appointment. Call at least 2 days before your appointment to make sure there is time to verify your appointment. You may call up to 90 days before your appointment.
Get reimbursed. After your verified appointment, the money for gas mileage will be loaded onto the Focus debit card. The gas money will be available within 14 business days of your appointment date.

Getting a Ride
If you don’t live close to public transportation or need more support, you may get a ride in a car or wheelchair assessable van. The ride may be shared with another member.
Call at least 2 days before your appointment. Let them know if you have a mobility device, a service animal, or a caregiver who will ride with you. Also, let them know what level of service you need:
Curb-to-curb: Your driver will meet you at the curb of your pickup and drop-off location.
Door-to-door: Your driver will meet you at the door or front desk of your pickup location. The driver will escort you to the door or front desk of your drop-off location.
Hand-to-hand: Your driver will meet you and a member of your care team at your pickup location. Your driver will bring you all the way inside at your drop-off location. Your driver will stay with you until someone from your care team takes you the rest of the way.
