Where to go for care:

If your life is in danger, call 9-1-1 or go to the emergency room immediately.

It is important to schedule a yearly check-up when you get a new doctor. This gives you a connection to a medical home that you can call when you are sick and need care right that day.

Primary Care Provider (PCP)

Call your doctor’s office for:

  • Checkups and preventative care

  • Blood work

  • Cold and flu symptoms

  • Managing chronic conditions

  • Headaches, back pain

Many clinics have appointments for same day care. This is the best place to get care if it’s not an emergency.

Urgent Care

Go to urgent care for:

  • Sprains and muscle injuries

  • Small cuts, burns, or injuries

  • Same day care when your doctor or PCP can’t see you, or you don’t have a PCP.

This is a great place for same-day care when your doctor’s office is closed.

Emergency Care (ER)

Call 9-1-1 or go to the emergency room for:

  • If your life is in danger

  • Feeling pressure on your chest or left arm

  • Severe neck, back, and head injuries

  • Hard time breathing

  • Extreme mental distress

If you go to the ER and your life is not in danger, you may wait longer for care.

To find the name of your doctor, or primary care provider, just look on the back of your ID card.

You can also search for a new doctor by looking in your plan’s provider directory. Just find your plan below and click on the contact button.

If you need to change your doctor, call your medical plan. They can also help you find a new doctor.

View ID Card

When you become a Health Share member, we will mail your Member ID Card to you within 30 days.

Your Member ID Card lists your health plans and their contact information. It also lists your primary care provider (PCP), that’s your main doctor.

Keep your card with you at all times for:

  • Making health care appointments

  • Checking in at your appointments

  • Picking up prescription drugs

  • Emergencies

Dental Benefits

If you are a Health Share member, you can get free dental care with the Oregon Health Plan (OHP). 

Taking care of your teeth helps keep your whole body healthy. Make sure you and your family see the dentist at least once a year!

You can find your dentist's contact information on your Member ID Card. You can choose a new dentist in network.

Frequently Asked Questions

  • Call your primary care provider to ask for a screening. You can have as many screenings as you need.

  • You can get a screening from a doctor. You can also get one from a nurse practitioner. Licensed physician assistants and continuing care providers can also give screenings.

    If you have any questions, contact Health Share Customer Service.

  • You may be referred (sent) to another provider. This may happen if a medical, mental health, substance use, or dental issue is found. The provider screening you will explain why.

    The other provider can help treat the issue. If you agree to the referral, they will help you make an appointment.

    If you need care coordination, Health Share and OHA will make sure you have it. Health Share can help you if need a referral for social services, education programs, and nutritional assistance programs.

  • After your screening, you can get your recommended services. These will take no longer than six months to get after a request is filed.

  • Yes, some services are not covered. These include the Supplemental Nutrition Assistance Program (SNAP), and other social service programs.

  • If you think you need to see a specialist or other provider, make an appointment with your primary care provider (PCP) first.

    Your PCP will decide which services and tests you may need, and refer you to a specialist or other provider if needed. However, you can see specialists for some kinds of care without seeing your PCP first.

  • To make an appointment for mental health services, call the mental health plan listed on your Member ID card. Tell them what kind of service you’re looking for and they will help you find a provider. You can also view a list of providers online and, for outpatient services such as therapy, contact them directly.

  • If you need a medical appointment, call your main doctor (also known as your primary care provider or PCP). They’re listed on your Health Share ID card. When you call your PCP for an appointment, be sure to:

    • Tell the office or clinic that you are a Health Share member

    • Give them the name of the medical health plan listed on your Health Share ID card ( CareOregon, Kaiser, Providence, or Tuality)

    • Give them your name and Member Number 

    • Tell them why you need an appointment

    Call in advance for appointments that are not emergencies. If you are sick and need to see someone the same day, tell the clinic’s staff person.

  • You should be able to get a routine or follow-up appointment within one month. If you have an urgent issue, you should be able to get an appointment within 48 hours.

     If you have questions or concerns about getting a timely appointment, please contact Health Share Customer Service for help.

  • Visit the Oregon Eligibility Portal (ONE) or call Oregon Health Plan (OHP) Customer Service at 800-699-9075 right away to let them know you are pregnant.

    Pregnant women are eligible for prenatal care including regular appointments with a pregnancy care provider, ultrasounds, labor and delivery, and care after giving birth.

    They also qualify for added benefits, such as prescriptions for glasses and more dental services.

  • Need a translator or sign language interpretation? You can have a translator at your appointment, free of charge.

    Just let your provider’s office or clinic know you need a translator when you call to make an appointment.

  • Yes. Oregon Health Plan members can request free trips to covered health care appointments. 

    Health Share members can call Ride to Care at 503-416-3955 (local) or toll free at 1-855-321-4899 (TTY/TDD 711) to arrange a ride.

  • Call the number for your dental plan listed on your Health Share Member ID Card.

    you find a dentist in your area or help you figure out whether you can continue to see your current dentist. 

    You can also look for a dentist on your dental plan’s provider directory.
