Renew Coverage or Update your Information

There are 2 ways to update your information or renew your Oregon Health Plan (OHP) benefits:

View ID Card

When you become a Health Share member, we will mail your Member ID Card to you within 30 days.

Your Member ID Card lists your health plans and their contact information. It also lists your primary care provider (PCP), that’s your main doctor.

As soon as you receive your card in the mail, look on the back and find the number for your PCP. Call and make an appointment for your yearly check-up so you can establish care with your new doctor or medial provider.

Keep your card with you at all times for:

  • Making health care appointments

  • Checking in at your appointments

  • Picking up prescription drugs

  • Emergencies

Need a digital ID card?

With My Plan you can:

  • See a digital copy of your HealthShare ID card on your phone, tablet, or computer.

  • Check the names and phone numbers of your health care providers for physical, dental, and mental health care.
