What Has Changed for Legacy Health PacificSource members:

Starting January 1, 2026

• Legacy Medical Group is now part of the CareOregon provider network.

• If you are a Health Share member and had a Legacy Health PacificSource plan, you now have a CareOregon physical health plan.

• Health Share will send you a new ID card in January.

• You will have a new Pharmacy number. If you need help to fill a prescription, call the CareOregon Pharmacy Help Desk at 844-245 -0449.
