Health Share sponsors health care related events that support our community’s needs, including health fairs, community events, fundraisers, conferences and provider related events. Please see below for information on the application process.
Organizations requesting event sponsorship support should complete the Sponsorship Application in full at least 60 calendar days before the start of the event.
Prepare to complete the electronic application by following the steps below:
- Prepare answers to our sponsorship application questions
- Have a copy of your W-2 form ready to upload
- If possible, also submit an event agenda. If you do not yet have one, you may submit one at a later time.
- Prepare any other supplemental materials you'd like to include in your application (sponsorship packets, flyers, brochures etc.)
Once you have everything prepared, please complete and submit your Sponsorship Application.
A decision from Health Share will be made within 45 days of submitting an application.
